Additional Instructions
Draw Times:LMWH: Pharmacy recommended draw time is 4-6 hrs after dose.
This includes Fragmin , Lovenox ,and Enoxaparin.
24 hr dose can be drawn just prior to dose.
UFH : 6hrs after dose or 6 hrs after any dose change.
After therapeutic, every 24 hrs.
3.2% NA Citrate. Critical fill tube (fill to line on label).
Keep stoppered.
Test must be ran within 2 hours or plasma must be removed and frozen.
Therapeutic Range
Unfractionated Heparin: 0.3 – 0.7 IU/mLLow Molecular Weight Heparin: Enoxaparin:
Anti-coag bridging (mechanical valves):
Peak: 0.5-1.0 units/ml
Anticoag bridging (mechanical valves) in pregnant patients:
Peak: 0.8-1.2 units/ml
Trough: ≥0.6 units/ml
VTE treatment (Peaks):
(Q12H): 0.6-1.0 units/ml
(Q24H): 1.0-2.0 units/ml
VTE prophylaxis in pregnant women:
Peak (Q12H): 0.2-0.6 units/ml
