Additional Instructions

Spin and remove from cells within 1 hour.
Designate last dose date and time.

Therapeutic Range

Gentamicin level, trough: 0.5-2 mcg/ml

Toxic: Trough >2 mcg/ml

Additional Information

Collection Time After Administration:
Traditional-intermittent regimen: should be drawn at steady state, after 2nd or 3rd maintenance dose
Peak: Collect 30 minutes after IV infusion or 1 hour after IM injection
Trough: 30-60 min prior to next dose

Extended-interval regimen (high-dose):
Random level: Collect 6-14 hours post dose, (pharmacist to use nomogram for dosing)
Trough: 30-60 min prior to second dose
